Goldilocks Mastectomy
Goldilocks mastectomy is a specialised reconstructive technique performed at the time of mastectomy using the patient’s own remaining breast skin and fatty tissue to create a smaller natural breast contour without relying entirely on implants or complex flap reconstruction.
This approach offers a balance between going completely flat after mastectomy and undergoing more extensive reconstruction procedures. For selected patients, it can provide improved contour, softness, and shape while avoiding implants or major tissue transfer surgery.
A/Prof Farid Meybodi provides personalised breast cancer surgery and reconstructive planning with a focus on cancer safety, aesthetics, patient comfort, and long-term wellbeing.
What Is a Goldilocks Mastectomy?
During the procedure, the breast tissue is removed while preserving selected skin and soft tissue that can be reshaped to form a smaller breast mound. Unlike traditional reconstruction techniques, Goldilocks mastectomy uses local tissue only and may often be completed in a single operation.
In some cases, the procedure may also provide a foundation for future fat grafting, implant augmentation, or additional revision surgery if desired later.
Who May Be Suitable?
Goldilocks mastectomy may be considered for:
- women with larger or heavier breasts
- patients wishing to avoid implants
- patients preferring a less extensive reconstructive procedure
- patients who may not be ideal candidates for complex flap reconstruction
Suitability depends on breast size, skin quality, cancer treatment requirements, and individual reconstructive goals.
Cancer Safety & Surgical Planning
Goldilocks mastectomy remains a true mastectomy procedure focused on complete removal of breast tissue while preserving suitable skin and soft tissue for reconstruction.
Surgical planning carefully considers:
- tumour location and cancer stage
- need for radiation therapy
- breast shape and symmetry goals
- long-term reconstructive options
A/Prof Meybodi works closely with multidisciplinary breast cancer teams to ensure treatment planning remains both oncologically safe and aesthetically thoughtful.
Recovery & Expected Outcomes
Recovery varies depending on the extent of surgery and whether additional procedures are performed at the same time. Patients may experience temporary swelling, tightness, bruising, and changes in breast sensation during healing.
The goal of Goldilocks reconstruction is to create a natural chest contour using the patient’s own tissue. Final breast shape and volume depend on the amount of remaining tissue, original breast size, and individual healing patterns.
Some patients may later consider:
- fat grafting (lipofilling)
- implant augmentation
- nipple reconstruction
- revision or symmetrisation procedures
Risks & Considerations
As with all breast surgery procedures, potential risks may include:
- bleeding or infection
- delayed wound healing
- fat necrosis
- asymmetry or contour irregularities
- scarring
- need for revision surgery
All reconstructive options, limitations, and expected outcomes are discussed carefully during consultation and surgical planning.
Compassionate, Personalised Care
A/Prof Farid Meybodi understands that decisions regarding mastectomy and reconstruction are deeply personal. Our goal is to provide thoughtful guidance, personalised surgical planning, and coordinated multidisciplinary care to help patients feel informed, supported, and confident throughout every stage of treatment, reconstruction, and recovery.
